Step 1

Initial Contact

​You, a friend, a loved one, or a care professional can reach out to our friendly team to request information about our services. If the information we provide sounds like something you would like to discuss further, our team will arrange a convenient time and date to meet with you. During this meeting, we can discuss your needs and how Habitus Homecare may be able to assist you.

When you make initial contact, we will ask for some basic information, including your full name, date of birth, home address, the best telephone number to reach you, and an email address.

Step 2

We Meet

Before we can deliver a package of care tailored to an individual, it is essential for a dedicated member of our team to visit their home for a comprehensive assessment. This assessment is a crucial step in our process. It involves a thorough evaluation designed to gather information about the individual's specific needs, preferences, and living environment. Our aim is to establish whether we can safely and effectively deliver the level of care required.

We will also go over your preferred days and times for care, ensuring we align with your schedule and lifestyle. Additionally, we’ll talk about how often you’d like support—whether it’s daily, a few times a week, or another arrangement that works best for you.

We recognise that there may be times when a home visit isn't possible due to various circumstances. Please know that at Habitus Homecare, we are committed to working with you and can make reasonable adjustments to the assessment process to accommodate your needs. 

Step 3

Care Begins

During the initial steps, we'll have gather all the essential information. This includes establishing the specific type of carer you feel most comfortable with, whether that might be someone with specialised training or simply a friendly companion.

Once we have everything in place and if you choose to proceed, we’ll introduce you to our dedicated care team. We’ll work closely with you to agree on a start date that fits seamlessly into your plans.

 

The Habitus Homecare management team is dedicated to staying involved even after your care begins. We will conduct regular reviews of your care plan, allowing us to reassess your needs as they change over time. We believe in open lines of communication, so we’ll check in frequently to ensure that you are not only satisfied with the care being provided but that we are also meeting your expectations and adapting to any new requirements that may arise.Your comfort and well-being are our top priorities, and we are here to support you every step of the way!

Examples of some of the questions that will be asked during an assessment

Personal Assessment

The personal assessment seeks to obtain a complete understanding of the individual.

​1. How is your hearing?

You will be asked whether you use hearing aids or are hard of hearing. This is so that carers can be made aware to speak loud and clear if the client needs it.

​

2. How is your speech?

The assessor will ask whether you have any speech impairments or difficulties so that carers can be made aware of before visiting.

​

3. How is your eyesight?

You will be asked whether you are registered blind, whether you wear glasses/contact lenses, and if you are prone to falls due to poor eyesight.

​

4. How is your mobility?

You will be asked whether you have any mobility issues, e.g., whether you get in or out of bed, use walking aids, or require hoisting. This is so that we can arrange for the appropriate carers and the number of carers to support you.

5. How is your comprehension?

You’ll likely be asked whether there are any known difficulties around communication.

​

6. How is your memory?

Your assessor will be interested to know whether you have been diagnosed with or are suspected of having dementia. Do they suffer from forgetfulness? Do they often lose things?

Do they ever get lost?

7. How is your mental health?

You will be asked whether you have any current concerns about your mental health and whether you have a history of mental health challenges.

Care Needs Assessment

The care needs assessment aims to establish the exact support the client needs during their home care visits.

​

1. Are you able to get in and out of bed unassisted?

You will likely be asked whether your loved one requires assistance or hoisting when getting in and out of bed so that appropriate support can be arranged.

​

2. Are you able to manage your own personal care, such as washing and toileting?

​​

3. Do you require support with taking medication?

Be prepared to be asked about the medication and whether you need any support or prompts to take it. They will likely ask whether you take your medication a specific way, at certain times, and how frequently you take it.

​

4. Does you require support with preparing meals and beverages?

Your assessor will ask whether you are able to cook and make drinks unassisted so that they know whether the carers will need to support you with these tasks.

​

5. Do you require support with household tasks?

In order to gauge whether you will need help with household tasks, they will ask whether you need support with cleaning, laundry etc.

​

6. Will you require assistance with attending social activities?

Please be prepared with your social activities calendar, as you will likely get asked whether you attend any regular clubs, coffee mornings, church, etc. This is so that they can tailor the care plan to provide support at these times.

7. Will you require support with attending appointments?

These could be doctor's appointments, hearing/eyesight tests etc.
